Registration Form for Child's Party - Details coming shortly (INVITATION ONLY)

Welcome to My Rainbow Kitchen. I'm excited to invite your child to this Birthday Party! I would appreciate it if you could fill out all the necessary information before the event.

During the party, children will be offered soft drinks or water and a few snacks such as crisps, tortilla chips, or popcorn, but please ensure they have eaten a snack or lunch beforehand.

Drop Off & Pick Up: Please drop off your children at the provided address. The party hosts and I will be there to welcome your child. Pick up will also be from outside the premises. If the party runs late, there may be a 10-15 minute wait.

Illness: Please do not send your child if they or anyone in your household is experiencing flu or COVID-related symptoms.

Dietary restrictions and preferences: Please clearly indicate on your booking form if your child follows a specific diet or…
